I (hereinafter referred to as the "User"), acting freely, of my own free will, and in my own interest, hereby provide UniPage, LLC, a company registered under the laws of the United States (hereinafter referred to as the "Company"), my express written consent to use my contact information to receive informational, marketing, and promotional materials.

The Company retains electronic logs (including IP addresses, timestamps, and web form data) as proof of consent.
